REMOVE DEPENDENT/UPGRADE

 

ALL dependent and upgrade changes are assigned a $3 change fee and require a minimum advance notice of 5 business days prior to your next draft date.  Any promotional prices or other specials you received when originally adding a dependent or upgrade may no longer be available in the future and all dependents will be charged an enrollment fee, if added back at a later date.  PLEASE READ any confirmation email closely as it does contain detailed information about your changes.  To proceed click the grey button below and/or refer to your membership agreement for more details.
